Course request

This form allows instructors to obtain a new course shell for development or a new section of an existing course. You can use this form to request a course for either WebCT 4.1 or CE 6.  If you are requesting a CE 6 course, please see the Start-of-Semester Procedures for additional information on how to prepare your course for students.

Designer Information:
Note: Designers will be given both designer and instructor access to the course.

  1. If other individual(s) need designer, instructor, or TA access to your course, indicate the type of access requested and give the person's full name, Pather ID, and e-mail address.
  2. If this course is cross-listed with other courses or sections, give all of the course ID's and sections to be cross-listed and indicate which section is the parent section.
  3. Provide any additional instructions or comments here.
